If the contractor business has employees, the DOPL application path may require a workers compensation insurance certificate. This is different from the general liability certificate and different from a workers compensation waiver.

A workers comp certificate generally shows that the business has workers compensation coverage in force. The certificate should be tied to the correct business name and should follow the current DOPL instructions for certificate-holder details.

If the business has no employees, DOPL may point the applicant to a Workers Compensation Coverage Waiver from the Utah Labor Commission instead of a workers compensation insurance certificate.
